Following is the form required for all event and publicity requests.  Please complete the form in its entirety.  Fields in red are required.  If a required field does not apply to your request, please select the appropriate response or type "na" in the field.  Please review carefully the "Terms of Agreement" at the bottom of the form.  Requests are reviewed each Tuesday at Staff Meeting.  You will be notified of approval or denial.

All drivers must be approved by the church office.  Bus drivers must have a current CDL.

TERMS OF AGREEMENT:

1. I understand that completing this form does not confirm the date or facility use.
2. I understand the event is confirmed when I receive confirmation from the church office staff.
3. I understand that picking up the church key/alarm code is my responsibility.
4. I will keep keys in my possession and will not duplicate.
5. I will return all rooms used to a clean, organized condition.

REGARDING VEHICLE USAGE:
1. I understand that picking up and returning vehicle keys is the driver's responsibility.
2. I understand that seat belts must be worn by the driver and ALL passengers AT ALL TIMES.
3. I will also complete the "Van/Bus Use Report Form".
4. I understand that the driver of the bus MUST have a current appropriate CDL.

FINALLY:
I understand that if access to the church building outside of normal operating hours is needed, or if I need use of the church vehicles, I MUST contact the church Office Administrator directly either by telephone or in person to coordinate pick up of the key(s).
